Module Overview
This module examines intelligence failures and the lessons learned from some of the most significant intelligence breakdowns in modern history. Although intelligence organizations devote enormous resources to collecting, analyzing, and disseminating information, failures still occur. These failures can result from incomplete information, analytical bias, organizational barriers, communication breakdowns, political pressures, technological limitations, or simple human error.
Students will analyze major intelligence failures such as Pearl Harbor, the September 11 attacks, and assessments regarding Iraqi weapons of mass destruction. Through these case studies, students will explore how intelligence information was collected, interpreted, communicated, and acted upon by decision-makers. Particular attention will be given to identifying the factors that contributed to failure and evaluating whether those failures were preventable.
The module also explores how intelligence organizations respond to failure through reform, improved coordination, enhanced analytical practices, and organizational change. By examining both successes and failures, students will gain a greater appreciation for the complexity of intelligence work and the challenges associated with producing accurate assessments in uncertain and rapidly changing environments.
