Overview
In this assignment, you will examine one of the most challenging aspects of intelligence work: intelligence failure. Intelligence failures can occur despite substantial resources, sophisticated collection capabilities, and highly trained personnel. These failures often result from a combination of collection gaps, analytical mistakes, organizational shortcomings, communication breakdowns, and decision-making errors.
You will analyze a significant intelligence failure, evaluate its causes and consequences, and assess what lessons can be learned from the event. By studying past failures, intelligence professionals can better understand how to improve future collection, analysis, and decision-making processes.

Assignment Topic
Select a significant intelligence failure and evaluate its causes, consequences, and lessons learned. Possible examples include Pearl Harbor, the September 11 terrorist attacks, intelligence assessments regarding Iraqi weapons of mass destruction, the Yom Kippur War, or another instructor-approved case.
Begin by describing the historical context of the event and explaining why the intelligence failure is considered significant. Discuss the available intelligence information that existed before the event and how intelligence agencies, policymakers, or military leaders interpreted that information.
Analyze the primary causes of the failure. Consider factors such as collection deficiencies, analytical biases, poor information sharing, organizational barriers, communication breakdowns, leadership decisions, technological limitations, or failures to recognize warning signs.
Next, discuss the consequences of the failure. Consider the effects on national security, foreign policy, military operations, intelligence agencies, public trust, or government decision-making.
Finally, propose specific reforms, solutions, or improvements that could reduce the likelihood of similar intelligence failures in the future. Support your recommendations with evidence from the case study and explain how they would strengthen intelligence collection, analysis, coordination, or management.
Conclude by evaluating whether the failure was primarily a collection problem, an analytical problem, a management problem, or a combination of multiple factors.
Acceptable and Ethical Use of AI
- You may use AI tools to identify intelligence failures, summarize historical background information, or clarify intelligence terminology. Any information obtained through AI should be verified using course materials and credible sources.
- This assignment requires critical evaluation of intelligence processes and decision-making. AI may assist with research and organization, but your analysis of the causes, consequences, and recommended reforms must be your own work.
